Complicación tardía en cirugía ortognática: fractura bilateral de placas y reintervención quirúrgica. Reporte de caso y abordaje con toxina botulínica

Resumen

La cirugía ortognática es fundamental para la corrección de las deformidades dentofaciales, promoviendo mejoras significativas en la estética facial, la función masticatoria y la calidad de vida de los pacientes. Sin embargo, pueden ocurrir complicaciones como fracturas de las placas de fijación, incluso en casos con una planificación quirúrgica adecuada y una ejecución técnica rigurosa. Este artículo tiene como objetivo relatar el caso de uma paciente sometida a cirugía ortognática que presentó fractura bilateral de las placas de titanio em el período posoperatorio temprano, asociada a bruxismo previamente no diagnosticado. El manejo incluyó una reintervención quirúrgica y la aplicación de toxina botulínica como enfoque complementario, con el objetivo de reducir las fuerzas musculares deletéreas y prevenir recurrencias. La discusión enfatiza los factores biomecánicos y fisiológicos implicados en estas complicaciones, así como la necesidad de una evaluación preoperatoria minuciosa y de estrategias multidisciplinarias para mitigar los riesgos. Este caso refuerza la importancia de identificar y manejar los hábitos parafuncionales, y destaca el papel de la toxina botulínica en el contexto de las cirugías ortognáticas, contribuyendo a resultados clínicos más estables y a la reducción del impacto de las complicaciones.
