Trauma pélvico em padrão ‘open book’: Revisão sistemática sobre diagnóstico e manejo emergencial

Resumo

O trauma pélvico em padrão “open book” representa uma condição grave e potencialmente fatal na emergência ortopédica, frequentemente associada a instabilidade hemodinâmica e elevada mortalidade. O diagnóstico rápido e o manejo emergencial adequado são determinantes para a sobrevida dos pacientes. O objetivo deste estudo foi realizar uma revisão sistemática da literatura para identificar e sintetizar as evidências disponíveis acerca do diagnóstico e do manejo inicial do trauma pélvico em padrão “open book”. Foram consultadas as bases PubMed, Scopus, Web of Science, Embase e Cochrane, utilizando descritores relacionados a “pelvic fracture”, “open book injury”, “emergency orthopedics”, “management” e “resuscitation”. Estudos publicados entre 2005 e 2025, em inglês, português e espanhol, foram considerados. Após a triagem, os estudos incluídos indicaram que o diagnóstico precoce, aliado à estabilização imediata com dispositivos externos (como o binder pélvico) e ao controle efetivo da hemorragia (embolização arterial ou cirurgia), são fatores-chave para melhorar os desfechos clínicos. Apesar dos avanços, a heterogeneidade nos protocolos e a ausência de ensaios clínicos randomizados de grande escala limitam a padronização do manejo.
