O Papel do teste cardiopulmonar de exercício no diagnóstico diferencial da dispneia em paciente com hipertensão arterial pulmonar primária

Resumo

Introdução: A hipertensão arterial pulmonar (HAP) caracteriza-se hemodinamicamente por uma pressão arterial pulmonar média (PAPm) >20 mmHg em repouso. Pacientes com Esclerose Sistêmica (ES) podem ser acometidos pela HAP e evoluir com prognóstico reservado. O padrão ouro para o diagnóstico é o cateterismo direito do coração, mas métodos não invasivos permitem o acompanhamento anatômico e funcional para avaliação da gravidade e do tratamento. O Teste Cardiopulmonar de Exercício (TCPE) avalia a condição aeróbica, o possível mecanismo responsável pela redução da capacidade de exercício e os marcadores de prognóstico, sendo reconhecido seu papel em pacientes com ES e HAP. Objetivo: O estudo tem como objetivo utilizar o teste cardiopulmonar de exercício no diagnóstico diferencial da dispneia em paciente portadora de hipertensão pulmonar primária. Metodologia: Trata-se de relato de caso associado à revisão de literatura. Conclusão: No caso clínico abordado, fica evidente a importância do TCPE no diagnóstico diferencial da dispneia e na avaliação de marcadores prognósticos, auxiliando na decisão terapêutica, a fim de proporcionar maior qualidade de vida e sobrevida dos pacientes.
